Are the barge decks and covers clear of grain, cargo, garbage, old lines and other foreign materials?

If the previous answer is "NO" was the material disposed of?

NOTE: ALL SUCH MATERIAL SHOULD BE REMOVED FROM THE BARGE DECK, PLACED IN TRASH BAGS AND PROPERLY DISPOSED OF AT THE WHARF

Is there a visible sheen on the water around the barge?

Is the barge listing, a known leaker or drafting more than normal or has other signs of water in the wing tanks?

If water is noted, identify the wing tanks and amount of water in each.

Is there an visible hull damage?

If the previous answer is yes, describe the damage and location

Are any pipes/hoses located on the barge leaking oil or any other contaminant?

If the previous answer is yes, state the location, amount leaked and whether the leak created a sheen on the water.

Are the drip pans clean or empty?

If not state the amount of oily water in the drip pan

Were the barge's wing tanks or hopper pumped?

If the answer is "No" Stop here and Submit.

Were the voids visually inspected for oily sheen, chemicals, solids, foam and other contaminants prior to pumping

Does the wing/void tank(s) water contain an oily sheen, chemicals, solids, foam and other contaminants?

Was a visual test for an oily sheen, contaminants and /or discoloration performed on the water surrounding the barge before, during and after the barge wing tanks were pumped?

Did the discharge water create an oily sheen or discoloration of the water?

Were the pumps stopped immediately and corrective action taken?

If yes, describe the corrective action taken
